	    This form must be signed by the permanent Faculty or Staff
	    person who is responsible for the machine.

	I agree to be bound by the terms of the Microsoft License

	agreements (a copy is on the CD's and on the Microsoft Web Site).

	Further, I agree not to copy these CDs, and to only install the

	software on the machine listed above.  I also certify that the

	above machine is a University of Minnesota Machine (Aerospace

	Engineering and Mechanics or Mechanical Engineering - Purchased

	through a University Purchase Order) that has an existing

	license to run a Microsoft Operating System (Windows 95 or newer

	- which is not and will not be used for any other machine).  I

	agree to remove the software covered under the Microsoft Site

	License from the above machine if either the ownership of the

	machine is changed or the University of Minnesota's agreement

	with Microsoft is terminated.  I also agree not to copy the

	license codes, nor to reveal them to anyone else for any purpose.

	Nor will I share the CD's I check out with anyone else.
